About Hidradenitis Suppurativa
Hidradenitis suppurativa (HS) is a chronic inflammatory skin condition that causes painful nodules, abscesses, and tunnels (sinus tracts) in areas where skin rubs together, most often the underarms, groin, buttocks, and under the breasts. HS often begins after puberty and can range from a few recurring bumps to widespread, scarring disease.
HS is frequently misdiagnosed as ordinary boils or "cysts that keep coming back." Early dermatologic evaluation matters, because effective treatment is available and earlier intervention can reduce scarring and complications.
Symptoms & signs to watch for
Common signs that may suggest hidradenitis suppurativa include:
- Painful, deep nodules in underarms, groin, buttocks, or under the breasts
- Abscesses that drain pus, sometimes with a foul odor
- Recurring "boils" in the same areas over months or years
- Sinus tracts (tunnels) and rope-like scarring
- Flares that worsen with friction, sweating, or hormonal changes
- Significant pain, drainage, or quality-of-life impact
Treatment approach
HS treatment is staged to disease severity. Early, mild HS often responds to topical and oral therapies, while moderate-to-severe disease commonly benefits from biologic medications and, in some cases, surgical procedures.
Treatment options your provider may discuss:
- Topical and oral antibiotics for flare control
- Hormonal therapies when clinically appropriate
- Biologic medications (e.g., adalimumab, secukinumab) for moderate-to-severe disease
- Intralesional corticosteroid injections for individual painful nodules
- Incision and drainage, deroofing, or surgical excision for selected lesions
- Wound care, lifestyle support, and care coordination for related conditions
